A psychiatrist is a doctor who specialises in diagnosing and treating mental health issues. If you're referred to one, here's what to expect during your visit and how they can help improve your mental wellbeing.

Mental health and psychiatry have been stigmatised for a long time, which sometimes makes people hesitant to see a psychiatrist, says Dr Prudence Molete, a psychiatrist at Mediclinic Legae. However, this is starting to change. “Often people feel that whatever they’re going through, they’ll just get over it on their own, but there are several reasons why seeing a psychiatrist may be necessary to help them deal with their situation,” she says.

Psychiatrist vs psychologist

  • Psychiatrists are doctors trained to assess a patient’s mental, cognitive, physical, and neurological health. They use this information to diagnose conditions and prescribe medication to treat them.
  • Psychologists are mental health professionals, but they are not doctors and may not prescribe medication. Like a psychiatrist, they’ll take your history and conduct assessments, such as psychological tests, to make a diagnosis. Psychologists mainly provide talk therapy, including approaches like cognitive behavioural therapy (CBT). Your psychologist may refer you to a psychiatrist if they think you need medication or further diagnosis.

Reasons to consult a psychiatrist

You may be referred to a psychiatrist by your GP or psychologist if you have severe symptoms that make it difficult to cope with everyday life. These can include:

  • Persistent depression and loss of interest in activities
  • Anxiety
  • Insomnia
  • Suicidal thoughts or self-harm
  • Difficulty controlling emotions
  • Mood swings
  • Feelings of worthlessness
  • Low energy and motivation
  • Poor concentration and memory issues
  • Relationship problems and social withdrawal
  • Financial stress
  • Workplace bullying and burnout.

“When symptoms persist over time and are affecting your day-to-day living, you’re likely to be referred to a psychiatrist,” says Dr Molete. She adds that substance abuse (drugs and/or alcohol) is a major factor that can make it difficult for many people to cope, often leading to them developing these symptoms.“

Symptoms may also be related to grief after the death of a loved one, a traumatic event, or diagnosis with a serious chronic illness such as HIV, kidney disease, or cancer.

What to expect at your first appointment

To see a psychiatrist, patients must be referred by their doctor or another healthcare professional.

An initial consultation with a psychiatrist usually lasts around 60-90 minutes. During this time, the psychiatrist will assess you in detail, learn about your history, and identify what may have triggered your symptoms, explains Dr Molete. The psychiatrist will also check for any other medical conditions that could be contributing to the mental health issue. After making a diagnosis, they will prescribe medication and recommend therapy options. Additional tests, like blood tests, may also be needed.

“The psychiatrist will explain the condition they’ve diagnosed and recommend lifestyle changes. They will also tell you about the medication you need to take, the possible side-effects, and what to do if you experience them,” says Dr Molete.

If necessary, and depending on your finances or health plan, your psychiatrist may refer you to other healthcare professionals, such as a psychologist, dietitian, occupational therapist, or social worker. If your condition requires it, you may also be admitted to a mental health clinic, especially if you're experiencing psychosis, mania, bipolar disorder, or suicidal thoughts. This helps stabilise you with medication and provides you with support from a team of specialists.

Dr Molete says outpatient psychiatric treatment can last anywhere from a few months to several years, or even a lifetime, depending on the severity of the condition and your ability to function.
